The 2008-2009 New Application
This copy of the 2008-2009 New Application is provided for informational purposes only, as the deadline has passed. The 2009-2010 New Application will be posted here by January 1, 2009, with a deadline of May 1, 2009.

Please note: The Phillips Scholarship is only awarded to permanent residents of New England and Mid-Atlantic states: CT, DC, DE, MA, MD, ME, NH, NJ, NY, PA, RI, VA, VT, WV. It is not available to international students.

2008-2009 Renewal Application
This copy of the 2008-2009 Renewal Application is provided for informational purposes only. The deadline for renewal was April 1, 2008, and no late applications will be accepted. The 2009-2010 Renewal Application will be posted here by January 1, 2009.

Change of Attendance Form

For temporary or permanent change of schools, or study abroad.
You may complete the Change of Attendance form in your browser, then print it and submit it.

Acceptance Agreement

Before recipients' award checks are mailed to their schools for 1st semester, an Acceptance Agreement must be completed, signed and received at Scholarship Office by June 27th, 2008. If students do not have access to their award packets, they may complete and print this web Agreement and mail it before the June deadline.

FERPA Release Form

A release form that may be needed in order for some colleges' financial aid offices to share information with the Phillips Scholarship. Most students will not need to use this form.

Home School Form

Students who were home schooled for a major part of their high school years should include this form with their New Application.

Sample Student Aid Report (SAR)

Here is a sample of the Student Aid Report (SAR) that must be included with your application. Notes on the SAR provide further explanation of required information for the financial portion of your application.

FAFSA Worksheet ABC

If you show entries in lines 40, 41 or 42 of the student section of the FAFSA or in lines 84, 85 or 86 in the parent section, you can print and complete this form to explain the entries.
